Please find my office form below.  The “Informed Consent” document will lead you through how I run my practice as well as your rights as a client and limitations to confidentiality.  A Health Insurance Portability and Accountability Act (HIPAA) Privacy Policy is also found in my “Informed Consent” document.  In order to download the “Informed Consent” document, select the underlined heading.  Please feel free to print this form prior to our first meeting.  If you prefer, you can also obtain a copy of this form when you come into my office for the first time.  If coming in as a couple, both people should complete separate copies of this form.
