Informed Consent

Please find my office forms below.  These “Informed Consent” documents will lead you through how I run my practice as well as your rights as a client and limitations to confidentiality.  A Health Insurance Portability and Accountability Act (HIPAA) Privacy Policy is also found in my “Informed Consent” document.  In order to download the “Informed Consent” documents, select the underlined headings.  I will have hard copies of these documents available when you come into my office for the first time.  If coming in as a couple or family, all people will sign the acknowledgment section of the form regarding the “Notice of Policies and Practices to Protect the Privacy of Your Health Information“, the “Office Information and Office Policies“, and “Telehealth Consent“.  Parents or Legal Guardians will sign the form for minor children.
